29 CFR 1926.1205(a): Before entry was authorized, the employer did not document the completion of measures required by paragraph 1926.1204(c) of this standard by preparing an entry permit: At the worksite located at 1601 SW MO-150 HWY Lee's Summit, MO: On or about May 11, 2024, an employee was exposed to chemical inhalation hazards. The employer failed to require an entry permit prior to authorizing employees to enter permit required confined spaces.
