29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. On or about April 10, 2024, in the Main ammonia engine room, the employer did not ensure dual relief valves on Low Stage Suction Accumulator (LSSA) were replaced and/or recertified according to their employed RAGAGEP including but not limited to IIAR Bulletin 110 (Start Up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems).
