29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a)TM Industries, LLC at 24211 412th Avenue, Artesian, SD 57314: On or about and at times prior to April 4, 2024, the employer did not ensure that electrical equipment was free from recognized hazards involving an extension cord set in that a 12-gauge extension cord was used to provide power to a Dewalt handheld angle grinder and had approximately five inches of the insulation removed with the inner conductors exposed. This condition exposed employees to electrical shock and burns.
