29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. On or about April 4, 2024 the employer BMB Construction LLC at jobsite located at 41 Gone Fishing Ln. Lakeside/Marblehead OH 43440, did not ensure employees were protected from falls while working from an elevated scaffold consisting of a pallet which was raised by a Gradall 534D9-45 rough terrain forklift (S/N: 0744030). One employee was exposed to falls of 10 feet and 9 inches (from the elevated scaffold pallet) or greater to the ground below.
