29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 20, 2024, the employer did not ensure that effective machine guarding was in place on the laminating machine at the pinch press area. The upper barrier guard did not extend to the framing of the equipment far enough to prevent employees from reaching into the ingoing nip point created by the two converging rolls during operation. b) On or about March 20, 2024, the employer did not ensure that effective machine guarding was in place on the laminating machine at the pinch press drop down roller area. No guarding was in place to prevent employees from the downward and closing action of the roller arm as well as the ingoing nip point created when the lower arm is in operation in the down position. The actuation of the device takes place with the use of a foot pedal allowing employees to reach into the hazardous area as it is initiated and takes place. c) On or about March 20, 2024, the employer did not ensure that effective machine guarding was in place on the laminating machine at the loading area. No guarding was present to prevent employees from the pinch point created by the loading table lowering onto its base as well as the automatic movement of the vacuum type loading robot. d) On or about March 20, 2024, the employer did not ensure that effective machine guarding was in place on the laminating machine at the stacking area. No guarding was present to prevent employees from the pinch point created by the loading table lowering onto its base. e) On or about March 20, 2024, the employer did not ensure that effective machine guarding was in place on the laminating machine at the stack roller area. The upper barrier guard did not extend out or down far enough to prevent employees from reaching into the ingoing nip point created by the roll rotating on the product during operation.
