29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) On or about March 12, 2024, with respect to an establishment located at 1001 Discovery Road, Green Bay, WI 54311, employees were exposed to potential struck-by hazards when the Toyota Model 7FGCU25, Serial #82957 was equipped with a split fork attachment but did not have a corresponding data plate to instruct the operator on safe load limits and load centers for the split forks. b) On or about March 12, 2024, with respect to an establishment located at 1001 Discovery Road, Green Bay, WI 54311, employees were exposed to potential struck-by hazards when the Toyota Model 8FGC55U-PRS, Serial #10187 was equipped with a Paper Roll Clamp attachment but did not have a corresponding data plate to instruct the operator on safe load limits and load centers for the Roll Clamp.
