On or about February 28, 2024 and at times prior thereto, on a table next to the #9 Komatsu 60t press, employees used an electrical pendant box that was installed at the end of a flexible cord and placed on a table which is not installed according to listing or labeling. b.) On or about February 28, 2024 and at times prior thereto, in the tool room, employees used an electrical pendant box that was installed at the end of a flexible cord and placed on a shelf which is not installed according to listing or labeling. c.) On or about February 28, 2024 and at times prior thereto, by the CNC Machine, employees used an electrical pendant box that was installed at the end of a flexible cord and hanging which is not installed according to listing or labeling.
