29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) On or about 2/26/2024, 5007 Starboard Street, Suite 7, Christiansted, VI 00820 - Pro Solar Office and Warehouse, the Toyota LP Model 5FGC15, Serial #11807, Capacity: 5000, sit-down power industrial truck had a broken bracket (Right side) and was not taken out of service, exposing employees to struck-by hazard.
