29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a. Hassia Auto-Stacker - On or about January 26, 2024, the employees engaged in changing the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 were not provided or assigned lock out tagout devices to allow each authorized employee to lock out the electrical, hydraulic, mechanical and gravity energy sources of the equipment prior to entering the danger area, the employees were exposed to caught in and struck by hazards.
