29 CFR 1910.119(j)(4)(ii): The inspection and testing procedures did not follow recognized and generally accepted good engineering practices. Location: Workplace production. On or about February 21, 2024, and at times prior, the employer failed to inspect or take actions to further evaluate or remediate: a) sections of uninsulated piping that exhibited signs of deterioration such as corrosion exposing employees to the hazard of ammonia inhalation. b) Valve inspection and maintenance to operate valves to assist in maintenance, repair and in an emergency situation; mitigate uncontrolled release and leaks exposing employees to the hazard of ammonia inhalation.
