29 CFR 1910.147(c)(5)(i):Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: At a facility located at 127 Claxton avenue, Carthage, MO 64836, employees were exposed to crushed by, amputation, and caught in hazards. The employer exposed the employees to these hazards by failing to provide them with the locks required to lock out the pneumatic and electrical systems of the PRO-PAL 3500 palletizer, located in the AST palletizer section of the facility.
