29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a.) Basement - On or about January 31, 2024, the employer failed to ensure that the sprocket wheel and chain for the crusher machine were enclosed with a guard. Employees operating the forklift in close proximity to the crusher machine to move glass were thereby exposed to amputation hazards created by the ingoing nip point(s). b.) Basement - On or about March 13, 2024, the employer failed to ensure that the sprocket wheel and chain for the crusher machine were enclosed with a guard. Employees using the forklift to move glass and walking in the area to access the employee breakroom were in close proximity to the crusher machine, and thereby exposed to amputation hazards created by the ingoing nip point(s).
