29 CFR 1910.178(q)(5): Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design. (a) Warehouse floor and exterior storage areas, 1420 Gardiner Ln, Louisville, KY: On or about 01/24/2023, and at times prior, the employer exposed employees to serious crushing injuries and death from tip-over hazards, in that the improper sized battery was installed in Toyota Forklift #16, model #7FBEHU18, which was 175lbs less than the minimum required battery weight for the PIT, while it was being operated by employees to move loads from the warehouse to the exterior storage area and cardboard compactor.
