29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) On or about January 2, 2024, and at times prior, the employer, did not ensure energy isolating devices such as a disconnect, were utilized to physically remove and isolate energy to the 230/460V 3-phase powered MAC Process Model MD40 Airlock, s/n: 1100310551-020-1. An employee suffered multiple finger amputations to their right-hand index, middle, ring and pinky fingers.
