29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: Site: 95 Anthony Street in Brooklyn, New York. a) On or about 1/3/2024, employees were working on maintenance repairs on a Wheel Loader when the machine moved after being turned on and struck an employee. The work area was not checked to ensure all employees had been safely positioned or removed from the hazard before the machine was turned on.
