29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. a) 159 Alexander Street, Yonkers, NY - Building C - 1st Floor - In the Main Residential Hallway: On or about 5/28/24, employees working/walking in this area were not protected against tripping and/or stepping into holes that had been created in the concrete floor slab and hole cut into a wooden floor hole cover. b) 159 Alexander Street, Yonkers, NY - Building C - 1st Floor - Inside Apartment 1001: On or about 5/28/24, employees working/walking in this area were not protected against tripping and/or stepping into holes that had been created in the concrete floor slab. In addition to abatement certification, the employer is required to submit abatement documentation for this item. Failure to comply will result in additional penalty of $ 1,000.00 as per 29 CFR 1903.19.
