29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about December 4, 2023, at the TPI (Third Party Inspection) area, employees were using grinders receiving power from extension cords that had missing ground pins where: a) Plugged into a 4X4 outlet box attached to the control panel. b) Plugged into a 4X4 outlet box on the floor by the TPI (Third Party Inspection) or platform, South yellow pole, near the booth.
