29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about November 20, 2023, at the Sinvo automated assembly machine cell (Line 1 - Wave 3 Pack Out), the employer by-passed the 22 integrated access door interlocks allowing employee(s) to access multiple point of operations to clear jams and to load material, to include but not limited to the energized conveyor, machine part presses and adhesive stations. This exposed employees to struck-by, caught-in and crushing hazards. (b) On or about November 30, 2023, and days prior, at the Sinvo automated assembly machine cell (Line 2 - Wave 3 Pack Out), the employer by-passed the 22 integrated access door interlocks allowing employee(s) to access multiple point of operations to clear jams and to load material, to include but not limited to the energized conveyor, machine part presses and adhesive stations. This exposed employees to struck-by, caught-in and crushing hazards.
