29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: 38 Jackson Rd, Devens, MA: On and before October 24, 2023, the employer's authorized employee training did not include the training of each employee to understand and utilize each unique energy control procedure before they were expected to use it. On and before October 24, 2023, some unqualified technicians-in-training performed tasks where they were potentially exposed to hazardous energy. On October 24, 2023, a Metrology Technician, who was not fully qualified as an authorized employee, received burn injuries from an uncontrolled pressurized steam release while removing a pressure transmitter from a machine. Later that day, the same not-fully-qualified technician reinstalled that same transmitter while the supervisor/trainer observed.
