29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure: a) Northbrook Behavioral Health: The hepatitis vaccine was not made available to an employee within 10 days of initial assignment and after the completion of training. The employee was hired in August 2023 and consented to receiving the vaccination, but the employer did not arrange for the administration of the first dose and as of 11/7/2023 the employee has not yet received the vaccination. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
