29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 26, 2023, an employee was observed working in an excavation that did not have cave-in protection installed. The employee worked in an excavation that was more than 5 feet in depth to locate a manhole and was exposed to hazards from falling and/or collapsing soil.
