29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level. a) On or about July 26, 2023, and times prior to, located at the R and S Loading Rack, employees working from an elevated Boom Trac platform with open hatches were exposed to a fall hazard when using harnesses and lanyards that allowed for a free fall more than 6 feet.
