29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. A) On or about August 2, 2023 at 100 Petty Rd, Lawrenceville, GA: A Nissan forklift was being used with a man-lift attachment. The attachment was used for activities including, but not limited to, lifting employees for circuit breakers. The company had not received the forklift manufacture's approval to use this attachment.
