29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: On or about July 13, 2023, at 140 North West Avenue, Youngstown, OH 44502, the employer failed to ensure employees performing tasks such as, but not limited to, repairing cranes, had been trained on safety related work practices, in that an employee was injured when there was an arc flash after equipment was re-energized. Employees must test for live voltage as part of their troubleshooting in the maintenance shop.
