29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about October 12, 2023, the employer did not ensure that the point of operations on the #3 Accurpress Model 71008 100-ton hydraulic press brake (Serial 4918) was guarded. The press was foot pedal activated and no guarding was in place to prevent an employee's hands from being between the dies when the press was cycled. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
