29 CFR 1910.263(g)(2): Handfed moulder(s) were not provided with either a belt-device or a hopper that extended high enough so that the hands of the operator could not get into the feed rolls: On or about June 28, 2023, the employer did not ensure employees were provided with either a belt-device or a hopper that extended high enough while operating the Bloemhof Dual Bolillo Sheeter / Molder (Model # 1-4-24 BL2 / Serial# 10H1463 - 115V, 1 Phase) so that the operator's hands could not get into the feed rolls. This condition exposed employees to amputation, struck-by and/or caught-in hazards.
