29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Wintech Takeoff on Calender 10 line: Employees were exposed to crush-by hazards when performing cleaning and set-up operations due to a lack of any lockout/tagout devices being affixed while engaged in those activities, on or about June 19, 2023. b) Shredder Line: Employees were exposed to struck-by hazards when performing repairs of the main shredder drive motor due to the lack of any lockout/tagout devices being affixed, on or about June 19, 2023. Abatement certification and documentation required.
