Text For Citation: 01 Item/Group: 004 Hazard:
29 CFR 1910.119(j)(5): Equipment deficiencies. The employer shall correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner when necessary means are taken to assure safe operation. The employer failed to repair an ammonia pipe hanger support above the Accumulator in room 901 located at 3910 Scanlon Avenue, Salina, Kansas. One side of the ammonia pipe hanger support was no longer connected to the mounting plate. Employees engaged in operating, inspecting, testing, and maintaining activities in the ammonia refrigeration system at the facility were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury.
