29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section: On or about June 5, 2023, employees were exposed to a struck by hazard while performing masonry activities on a commercial building located at 3750 West Market, Fairlawn, Ohio 44333. The employees were working from a supported scaffold with tools and materials that could have fallen on lower employees and the employer did not ensure that toe boards were installed to keep materials from falling on lower employees.
