29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service. On or about May 19, 2023, employees were installing clay tiles on the roof of the apartment complex under construction without inspections being conducted on personal fall arrest systems, exposing employees to fall hazards.
