29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. The employer did not certify annually that these operating procedures were current and accurate. Workers in the Cumene Plant at INEOS Phenol located in Pasadena, Texas, were potentially exposed to hazards when the employer did not certify in writing that the PP/Mix Tank Truck Procedure No. 8 was current and accurate on an annual basis.
