29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. Instance a.) On or about 3/08/2023, at the facility located in Enid Oklahoma on the platform of the evaporative condenser 3 (EC-3) on the roof the building #1, employees monitoring and/or checking conditions of the evaporative condenser were exposed to a fall hazard of more than four feet where the ladderway platform hole was not equipped with a self-closing gate or an offset. Instance b.) On or about 6/21/2023, at the facility located in Enid Oklahoma on the platform of the evaporative condenser 1 (EC-1) on the roof the building #1, employees monitoring and/or checking conditions of the evaporative condenser were exposed to a fall hazard more than four feet where the ladderway platform hole was not equipped with a self-closing gate or an offset. On or about 6/21/2023, at the facility located in Enid Oklahoma on the platform in the hydraulic room, employees monitoring and/or checking conditions of vessels were exposed to a fall hazard more than four feet where the ladderway platform hole was not equipped with a self-closing gate or an offset.
