29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion: a) On or about January 30, 2023, employees were exposed to fall hazards of up to approximately 145-feet while working from tube and coupler (pin lock and tube and clamp) scaffolding when their lifelines were attached directly to the scaffold and scaffold components.
