29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: JOB SITE: McDonalds, 1111 Bellefontaine Rd., Saint Louis, MO 63138, on or about 02/07/2023, the employer did not ensure that employees working in a JLG Telescopic Boom Lift (460SJ model / SN# 0300131175) at a height above ten (10) feet were connected into the boom or basket by use of a fall protection lanyard.
