29 CFR 1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who applied the device: On or about January 27, 2023, the employer failed to ensure that the lockout device applied to the electrical energy source, on the Stewart Mono-Flex Grid Bun Cooler, was removed by the authorized employee who applied the device. The equipment was re-energized after the lockout/tagout hasp was removed in disregard of the excepted provisions in 29 CFR 1910.147(e)(3).
