29 CFR 1928.57(c)(4)(i): Guards, shields, and access doors shall be in place when the equipment is in operation. a) Jobsite: 540 Church Rd., Avondale, PA 19311- On or about January 24, 2023, an employee conducting cleaning activities was exposed to crushing and caught-in-between hazards while operating the Peat Moss Dump Conveyor in that the exposed moving channel bars at the bottom of the conveyor were not guarded.
