29 CFR 1910.147(d)(5)(ii):If there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation did not continue until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed. This violation occurred on or about January 13, 2023 in the boiler area, the employer does not ensure that if there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation did not continue until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed this exposed maintenance to thermal hazards when performing maintenance tasks on the following equipment such as, but not limited to the Supreme boiler, model 8X-2000 with serial number 17536.
