29 CFR 1926.1053(a)(3)(i):Rungs, cleats, and/or steps of portable ladders (except step stools) and/or fixed ladders (including individual-rung/step ladders) were spaced less than 10 inches (25 cm) apart or more than 14 inches (36 cm) apart, as measured a) Jobsite: 1901 E York St, Philadelphia, PA 19125 - On or about January 10, 2023, the employer permitted employees to use a job-made ladder with rungs that were spaced approximately 16 inches apart thus exposing employees to slip and fall hazards.
