29 CFR 1926.452(w)(9): Caster stems and wheel stems were not pinned or otherwise secured in scaffold legs or adjustment screws. (a) At the jobsite, during painting: On or about 11/10/22, a Werner SRS-72 Steel Rolling Scaffold used for access while painting the ceiling was not equipped with manufacturer-supplied locking pins to securing the wheel stems in the scaffold leg.
