29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) In the die cast building, located at 7251 Zell Miller Parkways, Statesboro, GA 30458, on or about October 3, 2022, the employer exposed employees to caught-in, struck-by, and crushing hazards, in that the employer did not ensure all employees performing maintenance placed a lockout device on the machine or equipment in which they were working on.
