29 CFR 1926.1053(b)(5)(i):Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. a) Site- On or about August 14, 2022, employees working from a fiberglass extension ladder were not protected from a ladder collapse hazard where the ladder was erected with a horizontal distance from the top support to the foot of the ladder greater than one quarter of the working length of the ladder. The top of the ladder was supported against the wall at a height of 19 feet and the base of the ladder was 10 feet off of the wall. b) Site- On or about August 14, 2022, employees working from a fiberglass extension ladder were not protected from a ladder collapse hazard where the ladder was erected with a horizontal distance from the top support to the foot of the ladder greater than one quarter of the working length of the ladder. The top of the ladder was supported against the wall at a height of 21 feet and the base of the ladder was 12 feet off of the wall.
