1926.21(b)(2) The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a). On or about 10/04/2022, two employees were exposed to struck by hazards while standing next to a 3 foot by 4 foot by 6-foot-tall precast concrete box located within the swing radius of a Caterpillar 320C track excavator, and the excavator struck the box, causing it to tip over and strike one of the employees.
