29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a)Steinhaus Brothers, Inc. dba SBI Manufacturing at 47061 Phillip Street, Sioux Falls, SD 57108: On or about and at times prior to August 5, 2022, the employer did not ensure employees were protected from struck-by hazard related to forklift operation in that the company in that each powered industrial truck operator had not completed the training and evaluation specified in this paragraph (l). This condition exposed employees to struck-by hazards related to forklift operation.
