29 CFR 1926.651(k)(1):An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Corner of Zeke and Galm Rd, San Antonio, TX 78254: On or about August 2, 2022, and times prior thereto, employees were exposed to a caught-in hazard while working inside an excavation that was not inspected by a competent person to identify trenching and excavation hazards.
