29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Pipe Shop: On or about 07/20/2022, the unused opening on the right-side of breaker panel #7 was not provide with a cover thereby exposing employees to possible burns resulting from a potential arc flash. b) Pipe Shop: On or about 07/20/2022, breaker panel #6 had three unused opening that were not provided with a cover thereby exposing employees to possible burns resulting from a potential arc flash. c) Building #2: On or about 07/20/2022, an unused opening on breaker panel #15 was not provide with a cover thereby exposing employees to possible burns resulting from a potential arc flash. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
