29 CFR 1910.119(e)(3)(v): The process hazard analysis shall address: (v) Facility siting: On or about July 11, 2022, at the facility located at N4993 6th Drive, Westfield, Wisconsin, the last conducted Process Hazard Analysis, performed on October 5, 2020, did not include specific What-If /Checklist Worksheet questions covering all of the facility siting issues examples listed in the Scope section of the Process Hazard Analysis report. The Process Hazard Analysis questions only appeared to address the facility siting issues related to powered equipment contacting components of the ammonia refrigeration systems and did not address the other listed facility sitting issue examples included in the Scope section.
