29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Scaffold frames were used as a means of access. Location: a) Supported scaffold on ground level of the jobsite: On or about June 28, 2022, an employee was working on the fa�ade of the building while standing on the first tier platform of a two-tier-high supported scaffold. There was no safe means of access provided. The employee had to climb the scaffold frame and the crossbraces to get to the platform. The employee was exposed to falling up to approximately 6 feet to the lower level.
