29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: a) At the fabrication shop located at 1138-1 Pondella Road Cape Coral, Florida: On or about June 17, 2022, the employer did not ensure that the swivel rigging hook used to attach the jibboom to the slab lifting clamp to transport stone slabs, was inspected prior to use, and was removed from service when damaged or defective, exposing employees to a crush-by hazard. In that, the safety latch of the hook was opened, and the hook showed signs of wear and deformation. b) At the fabrication shop located at 1138-1 Pondella Road Cape Coral, Florida: On or about June 17, 2022, the employer did not ensure that the stone slab lifting clamp attachment used to transport stone slabs, was inspected prior to use, and was removed from service when damaged or defective, exposing employees to a crush-by hazard. In that, the rubber pads and shell show signs of wear and deformation.
