29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: At a worksite located at 614 W Johnathan Dr, Marionville, MO 65705: Employees were exposed to electrical shock hazards. This was demonstrated on or about August 17, 2022, and at times prior to, when a portable floor fan (model MC42) was connected in series to a cord and cable (extension cord), and the plug end for the portable fan was missing a ground pin.
