29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a) 221 Lincoln Ave, Elmwood Park NJ 07407: Employees were allowed to use the ladder jack scaffold to access the platform exposing employees to a fall hazard of approximately 12 feet. Stairway or ladder was not provided at personnel point of access as required, on or about 5/31/22 -
